How to build a Student activities booking form?

I would like to design a form for a school choice day. We have 500 students that get a choice of different activities for the day. Each activity has a limited number. What we need is a form that students can select the activity of their choice but when there are no places left the activity disappears. It would be useful too if the students could see how many places are left for each activity as students select them.

I have attached the form that I have designed but certain functions don't seem to be working. Students can change the number of places 

I also need a form that lists the choices that all students have made so we can register their attendance. 

1. To define activity limits you may use our appointment slots widget. Each time a students book for an activity the choices limit decreases.

2. To list the choices made by students the easiest would create a grid listing report:

Then embed that report somewhere in your form using an iFrame widget:

That way, a list of currently reserved time slots would be available
