JotForm in a sticky bootstrap collapse container is not working

Hello Jotform I am attempting to make a sticky collapsed container containing a form from Jotform. When I place the script calling the form and load my webpage, the form is not hidden inside of the collapsed container. Any suggestions would be appreciated. Thank you. 

I have checked your web page and see that the accordion seems to be working fine now. Were you able to fix the issue on the page?

I would recommend you to try embedding your form using the iFrame code instead, here is a guide that will help you getting it: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
