I am looking to create a landing page.

My vision is to have a headline, a video on one side and a box to capture name and email on the other side. After submitting the name and email, they get directed to the website, www.keyams.net. The promise of the capture is to send them a piece of information as described in the video and offered on a business card that showed the link to this landing page. 

You can add videos in your form using widgets.

You can setup your form with an autoresponder. This would send an email to your form users.
