My form does not register responses even though it's live

After entering required information in the form I click the button at the bottom of the screen and I get taken to a blank white screen. I have popups enabled on my computer. Please help!

I tested your form, but I could not figure out the cause of the problem, I even tried clearing your forms cache. However, I cloned your form(in your account), and the clone version works, could you please test it: https://form.jotform.com/83235818121150 

You may use the clone version instead and delete the other. Also, note that you can rename the title of the clone form, and the page title too: https://www.jotform.com/help/373-How-to-Change-the-Page-Title-of-the-Form
