Lifebook Form

    i want to create a Lifebook form that people can complete and then email to themselves, and/or their family or advisers. The information will be the most sensitive kind, and not for anyone other than the family or professional advisers, including things like bank account details etc. I want to make the template available, but once downloaded want the rest to be very private.

    A Lifebook is an end-of-days document detailing sensitive data plus end-of-life thoughts, intended to complement a Last Will. It is particularly intended to give password access to private accounts. The data is the most sensitive possible. Having a usable template makes collecting the data easier, but ensuring the data is then only shared as appropriate is critical.

    You have mentioned bank account details. Do note that we do not allow banking information inside our forms without using the Payment Integration as this would violate our terms of use.

    You can encrypt your form, so that the submissions would be encrypted.
