Hi again, I attempted to submit your form, and it was not possible to submit it:
However, I cloned it and cleared the forms cache, and the clone version works.
Please try the following:
1) Clear your forms cache: https://www.jotform.com/help/229-How-to-Clear-Your-Form-Cache
2) Test your form.
If the issue persists, try cloning and testing the clone version: https://www.jotform.com/help/27-How-to-Clone-an-Existing-Form-from-Your-Account
