I need to make a specific form

How can I have a file upload function and have that file upload form send it by email to the website owner as well as the product SKU of the page this is embed on

From what I understood you would like to have a form that will collect upload file and information from which site the form is uploaded (where the form is embedded).

You could do that with Upload field and Get form page URL:

That, of course, if the form page have Product SKU in it, if not, then some other methods should be used.

Do you know where the Product SKU is shown in the page and how?
