Forms not loading on my web site.

Thank you.  About a month ago some of the visitors to my site started telling me that the forms are not loading.  I can see the form on my laptop but not my Samsung Galaxy 9 S9+.  Here is a screenshot from my S9+.  Nik, thank you so much for your help + any other helpful feedback is very much appreciated. 

I have been testing the links you have shared and I was unable to replicate the issue, the form is showing up properly on my mobile device and using a S9+ emulator:

May you try using a different device? 

Also, try using a different internet connection and see if that helps.

Thank you so much for your time in looking into this.   I honestly don't know what the issue is but I'm going to continue to look into it.
