Use Code Block from your Avada theme to embed the form. If that won't work, try our iframe method within the code block.

Guide to the iframe code: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.

one of the reasons i'm contacting you is because traditional embedding doesn't work. adding the JS code in avada code block does nothing.
I will try the iFrame code but it's unfortunate as its a backward way of doing this.

I have Avada. I'll try it, and let you know.

Avada, default embed here http://www.fs-affiliates.com/sample-page/. It is working after enabling "Code Block Encoding". Please do the same to fix it.

Have you gone ahead and enabled Code Block Encoding in Theme Options > Advanced section of the theme. Refer to the guide here.
In addition, is the Iframe Embed Method not working as well? Try it and let us know how it goes.
