What is the form limit for each price plan?

regarding the price plan. is the number of forms the different forms created of the number of forms sent out? (to different candidates)

It is the number of forms you can create. For example, you can only create up to 5 forms with the free plan. 

The monthly submission limit is a different limit of the plan. For the free plan, you can only receive up to 100 regular submissions (submissions with no payment), and 3 payment submissions (submissions with payment). It is considered a payment submission if you have added a payment tool in the form like Paypal and received a payment to it.
