Form Trouble

I am having troubles with my form, when editing everything appears to be correct. But when you view the form 80 percent of the form does not show up. I have attached a few pictures to give you an idea. 

I am unable to replicate the issue you reported. 

I saw that your form has a lot of disabled broken conditions, so I assume that you were able to resolve the issue by disabling those conditions. 

Can you please confirm that the issue is resolved on your end as well?
