    Having problems with our embedded form

    Over the weekend we transfered the Hosting of our website. As of this morning, the accordian form that we had created with JotForm now shows one of the 3 forms expanded all the time.  Have tried to delete and re-embed with no success.  Wondering if someone there can somehow help me.  I've attached a PDF of the screen shot as well.

    Hi Nancy7200,

    As I've checked the form that was embedded into your website, I found that the two codes are conflicting which the first form isn't working. Please take a look at this video: http://www.screencast.com/t/4VFMgPcz

    First form is http://www.jotform.us/form/23054284773153

    Second form is http://www.jotform.us/form/23094085053147

    What I would suggest you to embed your second form using iFrame method if you want those two forms embedded into your website.

    The problem is we only want the ONE form (the 'Cloned' accordion form with
    all 3 forms embedded into one) to be the ONLY thing on our website. We
    created the Medical History form (and the other 2) and then someone from
    JotForm helped created the 'Cloned' form with all 3 forms. I'm not sure
    what I need to do to remove the 2nd 'Medical History' form that is showing
    up expanded on our site.
