autosuspend

while im trying to create my form it keeps logging me out and when I log back in it says autosuspend

Upon review of your account I found that it was automatically suspended (autosuspended) due to form: http://www.jotform.us/form/83324426137151 are requesting user credentials and social security number on plain field texts.

Gathering login credentials (username/password) infringes our Terms of Use, as this is only allowed through encryption along database administration; and our forms do not provide this kind of service.

Regarding the collection of social security numbers, you can request them by adding the mandatory widgets:

1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

2. Add a Signature Widget so your users can sign and agree to provide such sensitive information. 

I have deleted the infringing fields and reactivated your account, if you need to collect social security numbers you can add the mandatory widgets, I highly recommend you to add them as soon as you are able to login in order to avoid any further inconveniences to your account.
