    How can I create a form that will show me the url of the page it is posted on?

    I have a blog and will have many posts on it.  I want to provide a GET A QUOTE button but don't want the person to have to supply product detail except the quantity.  They would also imput their phone and email.  When I get the form the url will tell me which product it is.  This way i will not need to have multiple forms, just one.
