there seems to be something wrong. None of the forms are coming through with this information including dummie ones that I’ve sent myself where I know I’ve put the date in. Any idea what’s going wrong? Thanks Mel

I cloned your form and tested this myself. I was able to replicate the issue.

I checked the clone form (https://form.jotform.com/83444016782963).
Unique name of the date picker field is seen wrong in the notification email.

If you edit this on your form, the problem will be resolved.
