How can I send a pre-filled form for signature?

Hello,

I'd like to send a form to clients, pre-filled out with their information, so all they need to do is sign the form and submit it back to me. I have a form created, and tried using the placeholder option to pre-fill in the data...however, when it came time to print the form in PDF, the program didn't pick up on those fields (thinking they were still empty). Can you please advise?

Placeholder is basically a guide or hint so they're not actually a real value.

If you want to prepopulate fields, you can do that through URL parameter. Use this app to generate URL parameters https://prepopulate.jotform.io/

Thanks for the suggestion, and it worked, but partially. When I test it by signing one of the two signatures and then hit "submit application", I get a response that only one submission is allowed and it doesn't accept the submission. 

I'm not clear on what that means and how to fix the form.

This is happening because the Unique Submission is set to "Check cookies only". This will prevent duplicate or multiple submission from the same browser.

I'm trying to "pre-sign" the document prior to sending it to the recipient. As you can see, I added a PDF signature in the Header field, and used the subhead text as my job title...however, this doesn't show up on the PDF. Can you please help me with getting my signature and job title to prefill as well? Thanks!
