Is it possible to create evaluation form where the respondent is anonymous?

We have a hard copy supervisor evaluation form that people are hesitate to complete, i.e., feel that the ratings/comments will be traced back to them somehow.  We would like to create an online form that provides total anonymity, i.e., we have no idea who the respondent is and there is not electronic footprint that can identify him/her.  Is this possible to achieve with your program?

You could remove all Notifications from the form, and avoid collecting any personal data.
