Issues on payment methods for the form that I made

    I made a form called teams and divisions on my profile I'm sure you guys can see it at the bottom of that form right underneath player 8 it should have PayPal but it doesn't and I'm using this for my website could you help me out it says something about this isn't formatted yet use wizard what is Wizard?

    The Order form is not configured properly.
