I've posted this question before but did not see a response.
In my form I've set a condition so that if a person enters their First and Last Name into the name field (based on exact text), they should receive an email report of the survey back. Basically - if exact words "John Smith" is entered, send an email to John.Smith@email.com.
However, many (if not all) of these emails never get sent, and I double-check the names entered and they match.

I'm not sure what happened but I know I've answered this question before that I even had cloned your form to test.
I noticed that you have the condition for Rose Gnade entered twice. So you might want to consider checking your conditions list for double entries.
Another thing is that conditions for entering texts are case sensitive so you might want to make sure that capitalizations are exact. You might want to use a dropdown menu as an alternative.
Lastly, your dropdown menu with Philly (PA/SoNJ/DE), can you try changing that to something else like Philadelphia? as I tried when that is selected it's not sending an email while it works well when others are selected.
