Is it possible to create a form where the user enters the recipient's email address?

I'd like to create a form I can share with others but one that forces the user to enter the email address they want it to go instead of having to use a static email address

I'd be giving users a link to the form from a software program which would take submission data and write it into the database.

I hope my question makes sense.

This can be done by attaching a condition to send the notification to whatever address entered by the user. Here's what to do:

    1. Load the form into the form builder, open the notification and in the Compose Email section, copy to your clipboard the {tag} that is to the right of the Email question (if it's the form I think, that tag would be {email6})

    2. Close the notification and and click the Conditions button:

    3. Click Send an e-mail after submission:

    4. Click Next and create the following condition:

    5. Save the condition and save the form

Now the submission will be sent to whatever email is entered in that field.

Please let us know if you would like further assistance with this.

Thank you, it works just as I hoped.
Another question though. I noticed the URL now has "jotformpro" instead
    of "".
If I decide not to keep Jotform Premium will this URL change? And if so,
    to what?

The domain names in the URL is not actually base on your subscription plan. It is more on your location. jotformpro and .us are in the US area, .eu in Europe, .me in Asia.. If your subscription becomes Free, it will still use .us and will not affect your form in anyway.

So just to be sure I understand this correctly:
If I give someone this url... and later change my
    subscription back to free,
the above url will still bring them to the correct form?

Yes that is correct, your form won't be affected. The only way it would become affected is if you hit your submission or data limit per your plan and at that point it would display a message that your form has been disabled for the time being which would be displayed until you either upgraded or waited for the monthly reset of your plan but the form itself would still remain intact either way until then.
