How do I make my pdf look like this all the time?

I don't know what I did to make the beautiful one but the very next one I submitted l l  looked like crap again.  Please help me make my form look great like this one.

Your first post with the beautiful PDF looks like it was made on the old version of PDF Designer. 

Do you want to use the old version or the new version PDF Editor?

I recommend using the new version. It is easy to use and very easy to customize. May we know if you need help on the new version?

I need one that compresses into as little as pages as possible and still look good
