How can I insert this form in my Etsy account?

Do I have some kind of code?  Sorry but I am not that computer literate.  It works great in my website, but I have tried the HTTP but when they send it doesnt go anywhere.  What link do I need to use that will take them to the form that they can submit?

To get the form link, refer to this guide: Where to Find My Form URL. You can then go ahead and add that link to your website.

Alternatively, you can also embed the form on your website. The form will then show on your website and the user can fill it without leaving the website.
