I received an e-mail that a new client filled out an intake form, but none of my passwords are working to open it. Also, when I log into my account on jotforms, I cannot find this completed file either. Please help.

The form that you are looking for, I presume is https://www.jotform.com/82688785184173.
This form is created using the account with username: "cpwfteam". If this is your account, you can log in here and check your form and its submissions.

pictures... it shows up as error.

The form you are trying to view submissions/images is HIPAA compliant. You will only see submissions in that form if you are logged in using the account cpwfteam.
If you still receives error even after using the cpwfteam account to view the submission, please send us a screenshot so we can check and investigate the problem.
