Forgot username and password

    We forgot both username and password from which below form was created. If you can provide us with username and email, that would help us to retrieve our account. The form is not working and we need to fix it as soon as possible.

    The form belongs to the account username (jsharper) and it is registered to the email address (j***us.harper@dsnews.com)

    Hi John,

    Thank you so much for your reply. The email address you have mentioned belongs to our former employee which even does not exist now. Can you please change the email address to mukesh.maharjan@dsnews.com ? Please let me know if you need any other details for this process. This is very urgent as our subscription page is not working.

    The email address (mukesh.maharjan@dsnews.com) is already registered to the account (mukesh_maharjan). 

    Please try resetting the password and contact your email provider to give you access to the email address (j***us.harper@dsnews.com)

    Let us know how it goes.

    The above email does not exist now. That's the reason we have not been able to reset the password. It used to be the email address of one of our former employee. 

    Earlier today, I registered with mukesh.maharjan@dsnews.com in JotForm. I thought it would be necessary to have my account in JotForm before it can be used to retrieve the password for j***us.harper@dsnews.com. Please change the email address to mukesh.maharjan@dsnews.com so that I can have access to the form. Also, I have removed above email from this account. Let me know if any other information is required for this process.

    Please provide us with the complete email address of the account jsharper so that we can verify the account. Also, since your email address has already registered with another account, it is not possible to use the same email address with this account. However, we can move the forms from the account jsharper to your account mukesh_maharjan after verifying the account.
