and how do I revert back to the standard PDF?

I understand that you want to change the PDF report of your JotForm to the old report format. You may change the PDF report to the previous version from the form settings.

If you want to change it for all the forms in the account, you may switch by clicking on 'Switch to Old version' button on the right top corner in the new PDF editor screen.

Show me exactly what your referencing for the old report. I believe I tried that and it ask why. But nothing happened.
The Yes / No. I changed that and not reverting to your format for pdf
