Card form: Image Load Preview widget is not being displayed in form.

My "Image Preview" app is not working. I can't upload any files and it just shows up blank. Can you please help me?

I am sorry for the trouble caused to you. I did test your form and I am able to replicate it. I am not sure what caused this issue in your form.
