Need a professional form !

Dear CSE,

Greetings!!!!

I want to create a form, the problem is I want that in case a profile is being uploaded by a vendor it should be shown as a duplicate profile if uploaded by some other vendor after that. secondly when i receive any resume it should be received on my email address.

 This small help i need from your side in case possible please let me know!!
