    Hey Jotform,

    I was editing a from I was making for my website to handle costumer questions and it "AUTOSUSPENDED" my account. Could you please reactivate it?

    Upon review of your account, it was found automatically suspended by our Phishing Detector Tool (autosuspended) due to form: requesting login credentials (username and password) 

    Gathering this type of data infringes our Terms of Use.

    Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "Password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.

    We have deleted the infringing fields in order to avoid an autosuspension, your account has been reactivated.
