my account suspended

why my form was suspended after i create the password for that form. please assist.

Upon review of your account, it was automaticallys suspended by our Phishing Detector Tool (autosuspended) due to form: http://www.jotform.me/form/83532970856467 because it contains the word "Password" on plain field texts.

Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "Password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.

In order to avoid autosuspension, the field has been removed and your account is now active.
