How to embed a form to wordpress and integrate it with our software?

I am a dental office and look into getting some online form on my website.  It is a wordpress website.  www.daybreakdental.com

Once I have the online forms attached and the patient fills them out, can the information be linked to my Open Dental software or do I need to manually enter it?  Can I print out the completed forms?
