Cannot create form from scratch

I try to create a new form from a blank. When I click CREATE nothing happening.

We would like to apologize for any inconvenience. I have tried the issue you described on my end but I wasn't able to reproduce it.

However, when I checked your account I see that it already have the form you are trying to create in your screenshot. Was the issue already resolved? Otherwise, can you give us more details please?

1. What form are you trying to create — Classic form (All Questions in One Page) or JotForm Cards (Single Question in One Page)?

2. What browser were you using when you encountered the issue?

If possible, please send us a screenshot of the issue on your browser with its console opened.
