Date Picker Error

Hi,

I have downloaded the form successfully but I found that Date Picker doesn't work as the way it should be. When I clicked on the calendar icon, the date doesn't pop up. Can you please kindly help? Thank you.

I have tested your form and I can't seem to replicate the issue at my end. I have tested your form in Firefox and Chrome.
