Login failed - AUTOSUSPENDED

    Greetings and good morning,

    I am trying to create an IT intake form to securely onboard clients and collect account info like router passwords and other sensitive information. A little looking around leads me to believe my account may have been flagged as suspicious. What can I do to reactivate it?

    Upon review of your account I found that it was automatically suspended by our Phishing Detector Tool (autosuspended) due to form: http://www.jotform.us/form/83564892116161 requesting login credentials on plain field texts.

    Gathering this type of data infringes our Terms of Use.

    Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "Password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.

    The infringing fields have been deleted in order to avoid any further inconveniences like an autosuspension, your account has been reactivated.
