What is the total forms allowed per account type?

I have a total of 10 pages for my client intake form; in addition I have other forms I wish to create a template to use on my website. 

Im not sure how the yearly fee works and what am I paying for with a yearly or monthly fee?

Can I create additional forms at no charge using jot forms?

A free account allows you to create up to 5 forms, the Bronze plan up to 25, Silver up to 100, and for Gold plans there is no limit.

I am still confused...is it 5 free forms or 5 free pages? In other words can I create an Intake form that has 10 pages? Will this count as one form?

It is not per pages, it counts per form. You can create as many pages as you want on a single form and that will only be counted as one form.
