You are able to create your own custom fields if you wish.
Here is a sample of this:
Feel free to clone the form to see how it is made.
the Custom phone number field with the country code is made of:
Three "Free Text HTML" fields (label and the two dashes), and three "Text Box" fields (Country Code, Area Code and Phone Number).
I have used CSS for the positioning of the fields and also to hide the labels on the Text Box fields.
I then used the validation tools on top of the page to make sure that the user enters numbers and that the area code is 3 digits and phone number is 7 digits.
You can give it a try or if you would like me to set something like this on your form, simply let me know and I'll be glad to assist you!
