I am getting an "Form did not pass status check" error

    I am looking to use JotForm for collection application information for a K-12 professional development program but can't get past this error when attempting to submit a test application.

    Hi,

    I have smilar problem on my form. My user name CeRBeR and my form here

    How can i fix this.

    We are very sorry for the inconveniences caused.
    You account was suspended by our automated anti-phishing system. Please note that JotForm is not designed for login forms, that is why password fields look suspicious for our filter, we have re-activated your account. Please remove the password field from your form([Imported Form] Punapps - Powered by Google Form ID: 30227979583163 ) to avoid this from reoccuring.

    Just FYI, please avoid creating login forms, we do not support it. Our form can't work like a normal login form for website authentication purposes.

    I can't seem to replicate this issue. Is it happening from time to time? Can you please clear your browsers cache and recheck it again? Everything is fine on my side :)
