Error in "Condition" feature on my form: "Portal Rekrutmen"

Dear JotForm, there is an error on my form "Portal Rekrutmen". My set up about "conditions" cannot work properly.

Please your help immediately to fix it because so many "if" conditions in that form. Our responders very need that form today.
