How to show all texts fields in PDF form?

    I inserted two boxes of text in the Application I created, however when I did test run, the submission returned with all the information except the two text boxes I inserted.  These text are important part of the applications and when I covert them into PDF, it must be showing with the application.  Is there something I am not doing correct?

    Kindly enable the "Show headers and text" in your submission settings.
