I have been trying to add my Jot Form Feedback Form (one that you click on bottom of screen and it pops up) to a Drupal Template that I customized. I tried adding it to the page.tpl but also to each individual page and for some reason it keeps breaking the formatting of my menu (note: my menu is YUI module added into drupal). I have been able to add a regular form to the site no problems so the problem is isolated to adding the Feedback form.
Does anyone have any ideas how to input it without affecting my existing formatting?

I looked at your website and it seems to look alright, can you provide us a screenshot of the breaking up formatting of the menu?
As for inserting the form, you could have easily created a block and added the <script src="... ></script> in there. There is also an option where you can choose what pages you would like your feedback to show or not to show up.

When I add it in I get the following? I have tried adding it to blocks, to the page.tpl and also to the actual body of the script and I get the same thing?

I've been analyzing this further and it seems now clear to me. I apologize for getting this a bit mixed up. Please remove the script in all parts where you have added them. including the one inside the block. And then embed the feedback script only to page.tpl. Please place it just before </body></html>.
It would be great to see a screenshot of this page when you say it's breaking up while having the feedback inside the page.tpl so we can see how it looks like.

Can you show me how it looks like? I've been visiting your site and I find no feedback form embedded.
