Are you referring to the information that appears when you share your form via social media? If so, you can change the page title by following this guide https://www.jotform.com/help/373-How-to-Change-the-Page-Title-of-the-Form.
For the image, there's no direct way to do it, but the workaround is to add a "Text" field and put it above your form.

Please note that because of Facebook restrictions on displaying custom image, it is not possible to achieve your requirement. Here are the steps I would suggest you to follow:
#1. Download your form's full source code: https://www.jotform.com/help/104-How-to-Download-a-Source-Code-of-your-Form
#2. Add a new html file in your server and add your form's full source code in your html file.
#3. Add the Facebook meta tags in your form's full source code: https://developers.facebook.com/docs/sharing/webmasters
Please ensure to set your content values particularly the image as it is used as the thumbnail image on Facebook posts. Please check the screenshot below:

As I tried to check your form https://www.jotform.us/form/80597759969182 to share on my Facebook page, I see that there is no image being displayed. However, the form is displaying an image. As I check the issue further, the image seems to be added to the header group within the form theme.
Instead editing the full source code of the form, please follow the steps below:
1. Please inject the following CSS code to the form by following the guide How-to-Inject-Custom-CSS-Codes to remove the existing background image on the header.
.form-header-group {
background-image: none;
padding: 0px;
}
2. Now, you may add the image to header field from the field properties. Please see the screenshot below:
The uploaded image should displaying in your facebook post when shared.
