Why is the downloaded version not like the actual form online?

    Hi, I made a form, but when I published it, it doesn't look at all like the downloaded version.  Can you help me?  I've attached the link, and a screenshot of the downloaded version.

    The PDF and the online one is different since you have designed your PDF Form that way so the online version is not the same. You may change the background color of the online Form and then insert an image on top of the Form.
