A few people are having trouble completing our forms

    This is a complaint I'm getting too often:


    Chronic disease form Message I spent a long time trying to fill out the form. Some pages did not show up until I did "previous" all the way back. Then it wouldn't take one of my pages when it was totally filled out. When I repeatedly tryied to get it to accept the form, the entire set of "completed" forms disappeared. Very frustrating.

    Is there a way to capture when a person starts but does not complete a form so I can determine how often this happens?  If a few people are complaining, then probably a lot more are having the problem and this is something that is unacceptable and we both want to fix.  I'm not sure which form failed this time but I've contacted the individual to find out.

    We're sorry about that. Please gather the forms, and share the pages where they are embedded, so that way, we can inspect it. You are using a card form, and it is unusual for this to happen based on other's experiences. 

    Your last form ID here https://www.jotform.com/82534050789159 doesn't have many conditions to show/hide fields, so I don't think this would be the case, but again, we can investigate further. 

    To save the data, you may consider enabling Save and Continue, but users will have to click save in the form to do it. If you want the form to automatically save the data, choose Autofill. Guide below:

    I have copied the first page of the Chronic Disease Assessment. I am not sure if my information on that page will carry over. The entire assessment disappeared as I tried to go back to fill in the pages which were not shown to me initially. And when I had filled out one of the last pages completely, it “flashed and vibrated” saying I hadn’t completed it and would not go to the next page.

    If the IT team can locate my profile, please send it to me as I took quite a bit of time to fill it out, and I would like to salvage that information.
