How to build an appointment form that filters the users options as they answer questions?

With your forms, is it possible to build an appointment form that filters the users options as they answer questions? 

Ex: I have 3 doctors at location A, 2 at location B. 

Select a location, A or B. 

Answer: B

Select a doctor (only doctors at location B will be listed to choose from)

Doctor 1

Doctor 2

Make sense?
