There was no phishing attac. I want to copy a form for another account. Please relook my forms.

One of your forms is asking for personal information such as passwords and usernames. Please note that JotForm can not be used for any malicious internet activities. Perhaps we can consider re activating your account if you can provide us the reason why such form is collecting usernames and passwords.

Thanks for the response, your account has been reactivated however the form which triggered the Phishing filter had to be deleted.
In the future please avoid creating forms that constitutes phishing these includes collecting passwords.
