How to remove autofill on the form

I created a form in my files and sent it to someone for review and testing. Now that they have filled it out their information has populated into the form and every time I send it to someone their information remains. How do I make it so this does not happen? I built this form as a template that will need to be sent to all new agents or embedded into my website for completion. 

Please help!

I test and checked your form on my browser and it was not auto filled with data.

Try using another browser or use incognito or private mode. It should not auto populate the form.

What you were referring to was the Autofill feature.

You can disable it on your form settings.
