Hi, my account was suspended even i changed to new password, can you help to reactivate?

I did check your account and found that it is suspended as your form "BUSINESS PREVIEW" is suspected to use used for phishing activity.
I am escalating this thread to our form review team to review your form again. Someone form our form review team will get back to you on this soon.

Upon review of your account it was found that one of your forms was performing the collection of passport information on text fields. This type of collection is considered sensitive and it is only allowed if you use in the form the mandatory widgets:
