Captcha is displaying before submitting the form

Hello,

For some reason I am getting a captcha before I am allowed to submit this form.

I have tried submitting your form from my end and I was able to submit it without any issue. Are you still experiencing any issue with the form? If so, please try submitting the form from a different location or a device. You may try using your mobile device with data connection to see if you are still able to replicate the issue. I am getting this on every form.

I have also tried a different browser. I used internet explorer and am
still getting the same captcha. I have also filled out the main form and
previewed the answers, submit it and it also receives the same captcha.
There is only a few pieces of information that is being transmitted as
well. I have filled the entire form but only a piece of the information is
being saved.
