HIPAA compliant form.

    I manage a website for an optometry office.  Do I need a certificate on my whole site if I have the HIPAA compliant form embedded with Jotform? 

    Also, is there any other documentation I can share with him about how these forms are HIPAA compliant?  

    Right now I have neither...

    If the Protected Health Information (PHI) data is being collected only through the HIPAA compliant Jotform, then you do not require additional certification for the website. The data is collected and sent to your submissions in a HIPAA compliant manner.
