Form header image and e-signature aren't uploading properly for submission PDFs

Hey, hopefully somebody can help. Form has been working fine until today. Last few submissions have sent the form recipient a PDF file that doesn't have the header image, and the e-signature has been screwed up as well. This was working fine until today. Any assistance would be greatly appreciated! Thanks in advance! 

I am sorry for the trouble caused to you. I did test your form by sending  a test submission and I am not able to replicate the issue you are having. The PDF attachment in auto-responder email I received does display the header and signature correctly. Please check the screenshot below:

I would suggest you to please test your form again and get back to us if the issue persists.
