How do I link my jotforms with my DNS?

I'm trying to get my DNS connected to my jotform so I have my custom domain name, but the instructions are confusing! Can anyone explain it better? I use Iwantmyname.com as my host.

You have to do the configuration in your DNS admin panel or contact to your service provider and ask to redirect to the URL https://www.jotform.com/60558336062152, once your URL is loaded the user will be redirected to the form, only you or your service provider can do the redirection.
